Use tmux for stable Hermes auto-kickoff (avoid PTY freeze)
PTY relay freezes prompt_toolkit after the first turn. Prefer a detached tmux session, send short kickoff via send-keys, then attach. Keep PTY as fallback when tmux is missing.
